The terms and conditions listed below govern all coaching programs provided by Miverent. These terms and conditions must be agreed to by all customers and participants. These terms and conditions set out the full terms to which you must follow and adhere to at all times.

Miverent Football Academy respects your privacy and is committed to protecting your personal information.

Coaching Programs:

On the registration of any player to any Miverent Football Academy coaching program or clinic, the registering member agrees to the following terms and conditions:

- Payment is non-refundable once a program starts.

- By registering to any Miverent Football Academy coaching program, you agree to pay the full amount for the coaching program regardless of the amount of sessions you (or your child) actually attend.

- Only children pre-registered in a coaching program may participate.

- Notifications of any cancelled sessions will be via our TeamApp and/or direct contact with registered parents/guardians, if timings allow. In the case of unplayable weather or natural disasters where sessions are cancelled Miverent Football Academy will refund any sessions cancelled or offer a credit of the same value as paid for the session.

- No "make up" sessions are provided for children who do not attend the program for any reason.

- No refunds are due for sessions that are not attended. If you (or your child) are unable to attend due to serious injury or illness, a hospital or specialist doctors document must be supplied to Miverent Football Academy to assess the reasons for any potential refunds. Please note, refunds are not guaranteed and will be assessed on each individual circumstance.

Parent/Guardian Agreement for Junior Coaching Programs:

I accept full responsibility for the conduct and welfare of the player named whilst participating in any of Miverent Football Academy activities.

I agree that the player(s) and any parents/guardians relating to the player will abide by the Terms and Conditions and Code of Conduct of Miverent;

I give permission for urgent medical attention to be supplied if the player is injured and a parent/guardian is not available to consent;

I understand it is my responsibility to advise Miverent Football Academy if there are medical issues that will affect the player’s ability to safely participate in any coaching activities.

Participants with Medical Conditions:

Miverent Football Academy staff do not supply and/or administer any form of medication to any person attending Miverent Football Academy sessions.

If a person has any form of medical condition that is deemed serious or potentially life threatening by a doctor, then Miverent Football Academy must be provided with a signed "approval to participate letter" from a doctor, before that person can participate at Miverent Football Academy.

Miverent Football Academy does not take responsibility for any form of medical treatment other than emergency first aid while awaiting Ambulance or other medical professional service to the individual. If Miverent Football Academy staff deem it necessary to call an Ambulance for any individual involved with Miverent Football Academy programs, then they will do that and any costs associated with the Ambulance or associated medical treatment is the sole responsibility of the injured individual or guardian(s).

If Miverent Football Academy staff believe that a child attending an event organised by Miverent Football Academy is unwell then the parent / guardian will be advised as soon as practically possible, and requested to collect the child.

Code of Conduct:

Anyone identified as being abusive and/or violent or self endangering whilst participating in Miverent Football Academy programs may be banned from entry into Miverent Football Academy programs.

Any illegal acts may be referred directly to the Police by Miverent Football Academy.

Any damaged caused to the facilities being used by misconduct that results in repairs, all costs will be charged to the person(s) responsible.

Liability and Insurance:

All participants understand that they participate, attend and use the facility, they do so at their own risk. Miverent Football Academy and its owners, employees or agents shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participation or attendance at a Miverent Football Academy program. All participants and parents assume full responsibility for all injuries and damages which occur in or about any programs on a Miverent Football Academy location. The hereby declare any personal injuries or property loss not to be the responsibility or fault of Miverent Football Academy staff or location.

All persons entering a location of Miverent Football Academy release and discharge Miverent Football Academy, its employees and agents from any claim, suit, demand, expense or cost in respect of any injury or illness arising out of or in connection with their participation in activities provided by Miverent Football Academy.

Permitted Activities:

No commercial activities of any/all form are permitted at a Miverent Football Academy program other than those conducted by Miverent Football Academy, unless otherwise authorised by Miverent Football Academy in writing.

Any person(s) or organisation(s) identified as conducting such activities without the written authority of Miverent Football Academy may be banned from Miverent Football Academy programs.

Consent: 

I the undersigned parent or guardian or participant do hereby grant authority to the staff at Miverent Football Academy to render any judgement concerning medical assistance or hospital care in the event of an accident or illness during my absence. 

I do hereby authorise Miverent Football Academy and its staff to utilise any and all photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films.

I do hereby fully understand the terms and conditions outlined in this document and agree to all and any information outlined.
